Please note the following contest rules and conditions: 1. All AAPCC members, volunteers, employees and consultants are eligible to enter the contest. 2. Contestants may submit an unlimited number of entries. 3. All name submissions must include the word ‘Poison’ in some form. 4. Submissions may not use the phrase ‘TESS’. 5. Entries must be submitted by email to members@aapcc.org, with the subject line “Contest Entry”, using the official entry form, and must include:
6. Entries must be dated by 11:59pm on Monday, 9/4/06. 7. Entries will not be read or reviewed until after the submission deadline. 8. All decisions regarding the outcome of this contest are at the sole discretion of the AAPCC Board of Directors. 9. The AAPCC Board of Directors will receive all submissions without each contestant’s identifying information. 10. The winner of the contest will be announced at NACCT and will receive an iPod Nano. AAPCC exclusively reserves the right to substitute a prize of equal or greater value. 11.
